When booking your appointment, please have the following information ready:
- Your preferred time and Location
- Health Fund details (If applicable)
- Medicare card number
- Health care card details (pensioner/DVA etc)
- Your referral letter to confirm procedure information
- Your Credit Card in order to pay for your Consultation, Prep Appointment, Co-payment or Excess over the phone. We accept VISA and Mastercard, and VISA/Mastercard Debit Cards.
